Please provide the following information:

Applicant information

     - date of application

     - organization name, address, phone number and website address

     - legal status of organization, date 501c3 was granted, and EIN

    - general purpose of grant in 1-3 sentences 

     - grant amount requested (please note that the vast majority of WLFF grants are $5,000 - $15,000/year)

     - time period of requested grant

     - contact name, address, email and phone number

 

Proposal summary (one paragraph maximum)

     - summary of proposed project activities

     - why your organization is requesting this grant, description of anticipated results, how funds would be spent and how the proposed project contributes to your overall mission

 

Narrative description (not to exceed 2 pages)

     - background of organization, listing  of staff and board, and description of the work of the organization

    - project description, goals, objectives, timeline, methods and approaches used, and expected outcomes and results

     - population to be served and how they will benefit from project

     - partners, collaborators or other funders

     - how this request falls within the current geographic and substantive focus areas of the WLFF

 

Attachments

     - most recent annual statement with financials of the organization

     - detailed budget for the project and current organizational operating budget

     - copy of current IRS determination letter indicating tax-exempt status
